Write down what is included, and lock it
The one step that makes every later request answerable.

What’s included is an itemised, versioned record of what you agreed to deliver: the items themselves, your assumptions, what is explicitly out, the acceptance criteria, and how many revision rounds the fee covers.
Important: An unlocked draft protects nothing. Both the revision meter and the out-of-scope check read locked records only, so a project with a half-written draft behaves exactly like a project with no scope at all.
Locking is not final. A signed change order supersedes it with a NEW version rather than editing the old one, so the agreement and the record never diverge and you can always show what was true at any point.
